Warfarin vs heparin for pharmacology - the key differences?
Onset, route, monitoring and reversal keep tripping me up.

Heparin: parenteral, immediate onset, monitor aPTT, reverse with protamine. Warfarin: oral, delayed onset, monitor INR, reverse with vitamin K/FFP. Warfarin crosses the placenta; heparin does not.
