Everything an international medical student needs to start: the three Steps explained, exactly how to apply, a proven prep strategy, high-yield concepts with images and videos, and challenging mock questions.
🎯For Indian & international students — can you apply?
To sit Step 1 or Step 2 CK you must be officially enrolled in — or a graduate of — a medical school listed in the World Directory with an ECFMG Sponsor Note. There is no rule that lets you take Step 1 'after first year': because Step 1 tests the basic sciences, students normally take it once those subjects are complete (commonly 2nd–3rd year of MBBS). Full ECFMG Certification also needs four credit years and graduation, so it is finished after MBBS. Always confirm current rules in the USMLE Bulletin of Information and on ecfmg.org.
Senior Mentor Rounds for USMLE
Two minutes with Dr. Amy
Read each vignette like a calm ward round: localise the problem, protect against danger, then choose the best next step.
Dr. Amy is a MedPulse IQ teaching persona. Narration uses an available English voice on your device; voice quality varies by browser.
Physiology round
Read compensation before it fails
A changing patient speaks through physiology before the diagnosis is certain. Read the trend, identify what the body is defending, and decide whether compensation is still effective.
Ask yourself
01Which variable is disturbed?
02What compensation should I expect?
03What finding would tell me that compensation is failing?
Vignette lens
Treat every value as part of a trajectory, not an isolated fact.
You do not need every answer at once. Make the next safe reasoning step clear.
Curated from the Experienced Doctors teaching guides. Educational support only; use current primary references, official exam sources, local protocols, and qualified supervision.
Tests: Basic sciences — anatomy, physiology, biochemistry, pathology, pharmacology, microbiology, immunology and behavioral science — organized by system and process.
Format: One 8-hour day: fourteen 30-minute blocks of up to 20 items each, up to ~280 items in total, plus 55 minutes of break time and a 5-minute optional tutorial (format in force since 14 May 2026).
Scoring: Pass / Fail only (since January 2022).
When (India): Usually after the pre- and para-clinical years, once the basic sciences are covered — commonly around 2nd–3rd year of MBBS. Not realistically after just first year.
Step 2 CK
Tests: Clinical knowledge — diagnosis and management across medicine, surgery, pediatrics, obstetrics & gynecology, psychiatry and more.
Format: One 9-hour day: sixteen 30-minute blocks of up to 20 items each, plus 55 minutes of break time and a 5-minute optional tutorial (format in force since 7 May 2026; previously eight 60-minute blocks).
Scoring: A 3-digit numeric score — the score most residency programs now weigh heavily.
When (India): During the clinical years or internship, after solid clinical exposure.
Step 3
Tests: Applying knowledge for unsupervised practice — patient management, plus computer-based case simulations (CCS).
Format: Two days: multiple-choice blocks, then more questions with interactive CCS cases.
Scoring: A 3-digit numeric score.
When (India): After ECFMG Certification — usually taken during or after the first year of US residency.
How to apply — step by step
1
Check your medical school is listed
Confirm your college appears in the World Directory of Medical Schools (wdoms.org) with an ECFMG 'Sponsor Note'. Without that note, its students and graduates are not eligible for ECFMG Certification.
2
Create your MyIntealth (ECFMG) account
Set up an account, complete identity verification, and submit the Application for ECFMG Certification via the MyIntealth portal (ecfmg.org).
3
Register for Step 1 & Step 2 CK through FSMB
International students apply for the USMLE through the FSMB (usmle.fsmb.org). Choose your eligibility period and pay the fees.
4
Schedule at a Prometric center
Once your scheduling permit is issued, book your date and test center at prometric.com. Centers are available in India.
5
Meet the skills requirement (an ECFMG Pathway)
Satisfy the clinical- and communication-skills requirement through an ECFMG Pathway, which includes the OET Medicine English test (Step 2 CS was discontinued).
6
Graduate, get certified, then Match
ECFMG Certification is completed after graduation (your diploma is verified). It is required to take Step 3 and to enter US residency through the NRMP Match (applications go via ERAS).
The USMLE is organized by body system. Master the basic sciences system-by-system — the MedPulse IQ Concept Library and Dissection Guide map neatly onto Step 1.
Think in clinical vignettes
Questions are long scenarios. Practice reading a vignette, spotting the key clue, and reasoning to the single best answer.
Use spaced repetition
Revisit high-yield facts and mnemonics regularly. Daily Revision and Brain Gym keep them fresh over months of prep.
Practice, then review everything
Question banks are the highest-yield resource. Review every item — right or wrong — until you understand exactly why.
Simulate the real day
Take timed, full-length mocks to build stamina for an 8–9 hour exam, and practice pacing (about 90 seconds per question).
Weight your effort
Pharmacology, pathology, physiology and micro/immunology carry heavy weight — but don't neglect biostatistics and ethics.
USMLE-style mock test
🆕 2 fresh today
Practice from 2260 single-best-answer questions, including 1,363 items written to 130 NBME item architectures and cleared by three independent accuracy reviews. Blocks work like the real thing: move freely between questions, flag items, strike out eliminated choices, and change answers until you end the block, with laboratory values and a calculator always to hand. Timed mode holds feedback until the end; Tutor mode teaches immediately. Every block closes with a weak-system breakdown and the full teaching explanation for all items.
Start a question block
Blocks mirror the real examination: move freely between questions, flag items to revisit, strike out choices you have eliminated, and change answers until you end the block. Laboratory values and a calculator stay available throughout, as they are at the test center.
