The shape of a collection inside the skull is dictated by anatomy, which is why this is a first-year topic and not just a neurosurgical one. The dura is firmly attached at the sutures. A collection outside the dura therefore cannot cross a suture, and is pushed into a biconvex lens. A collection deep to the dura can spread widely and follows the surface as a crescent.
