Reduced bone mass and quality that increases fracture risk — often silent until a fragility fracture (wrist, hip, vertebra). Diagnosed by a fragility fracture or low bone density (DXA T-score ≤ −2.5).
Postmenopausal women and older men; risk with corticosteroids, early menopause, smoking, alcohol, low body weight, family history, and secondary causes (hyperthyroidism, hyperparathyroidism, malabsorption, hypogonadism).
First-line antiresorptive
Caution: Ensure vitamin D/calcium replete first; counsel on dosing/administration.
