Who gets it

Heart, liver or kidney failure (transudates); pneumonia (parapneumonic), tuberculosis (very common in India), malignancy and pulmonary embolism (exudates).

Red flags — escalate / refer

A large effusion with respiratory distress, empyema/sepsis, or haemodynamic compromise → urgent drainage and admission.
