Excessive bleeding after birth (≥500 mL vaginal / ≥1000 mL caesarean, or any amount causing instability) — a leading cause of maternal death. Uterine atony is the commonest cause.
All deliveries; risk with prolonged/augmented labour, multiple pregnancy, polyhydramnios, previous PPH, retained placenta and coagulopathy.

Caution: Avoid ergometrine in hypertension/pre-eclampsia; use carboprost with caution in asthma.
