A rare, life-threatening exacerbation of thyrotoxicosis — high fever, tachycardia or atrial fibrillation, agitation/delirium, vomiting/diarrhoea and heart failure. It is a clinical diagnosis; treat before labs confirm.
People with (often undertreated) hyperthyroidism/Graves', precipitated by infection, surgery, trauma, an iodine load (contrast), childbirth, or stopping antithyroid drugs.
Adrenergic control + block synthesis
Caution: PTU is often preferred in storm (also blocks T4→T3 conversion); avoid aspirin (displaces T4).
