Infection of the urinary tract — lower (cystitis) or upper (pyelonephritis). Escherichia coli is the commonest cause; distinguish uncomplicated from complicated infection.
Women (short urethra) most commonly; also pregnancy, catheters, diabetes, obstruction/stones, and men (usually treated as complicated).
Uncomplicated cystitis (first-line)
Caution: Avoid at term in pregnancy and in significant renal impairment (eGFR <45).
