A Word From Your Senior
An impressive radiograph can distract from the real priorities: life, limb perfusion, nerve function, skin, infection, pain, mobility, and the patient's future function.
Assess Before and After Every Intervention
Use trauma priorities first when mechanism or physiology warrants it. Examine and document skin, wounds, deformity, swelling, tenderness, compartments, distal pulses, capillary refill, motor function, sensation, and joints above and below. Repeat and timestamp neurovascular findings after splinting, reduction, transfer, or clinical change.
Bone heals through overlapping inflammatory, reparative, and remodelling phases. Stability, blood supply, soft tissue, infection, smoking, nutrition, age, disease, and medicines influence outcome. Not every fracture needs surgery; not every displaced fracture can safely wait.
Joints and Imaging
Arthritis is not one disease. Distinguish traumatic, degenerative, inflammatory, crystal, infectious, and referred causes. A hot swollen joint can represent septic arthritis and requires urgent assessment; normal early blood tests or absence of fever cannot safely exclude it.
Plain radiographs may miss occult fracture and soft-tissue injury. Imaging choice follows mechanism, examination, age, and consequence of a missed lesion.
Bedside Case
A patient with a tibial fracture develops escalating pain despite analgesia, pain with passive stretch, and altered sensation. Treat possible compartment syndrome as a time-critical emergency: remove external constriction as appropriate, urgently escalate for senior orthopaedic assessment, and do not allow a present pulse to falsely reassure you.
Accuracy Guardrails
- A palpable pulse does not exclude compartment syndrome or important vascular injury.
- Do not repeatedly manipulate a limb without a clear plan and analgesia.
- A cast can cause pressure injury or neurovascular compromise; provide explicit return precautions.
- Prostheses and fixation devices solve mechanical problems but introduce infection, loosening, wear, and failure risks.
- Suspected open fracture requires urgent protocol-led antibiotics, wound care, tetanus assessment, and specialist management; do not probe the wound casually.
