A doctor tells a diabetic farmer, "Avoid carbohydrates, take metformin 500 mg BD after food," and moves on. The farmer goes home confused. Another doctor asks what he usually eats, explains in simple words with his own foods, draws a quick picture of his plate, and asks him to explain the plan back ("teach‑back"). The second farmer actually changes his diet. Same knowledge, very different communication.
