In real life
A pregnant woman is seen by the ANM at the Ayushman Arogya Mandir in her village. Her BP is high, so she's referred to the PHC, then to the CHC (a First Referral Unit) where an obstetrician confirms pre‑eclampsia. At 36 weeks, she's admitted to the district hospital for delivery. After discharge, the ASHA follows her at home. Each rung did its job.
