The reasoning: active infection versus old antibody
Active infection is demonstrated by urea breath test, stool antigen or biopsy-based testing. Serology only shows exposure and can remain positive after cure. Non-invasive tests are falsely negative if PPIs, antibiotics or bismuth suppress bacterial load; hold PPI about 2 weeks and antibiotics/bismuth about 4 weeks when clinically safe.
