HBV natural history is dynamic. Practical treatment decisions combine HBV DNA, ALT, fibrosis stage, cirrhosis, age, family history of HCC, pregnancy context, extrahepatic disease, coinfections, planned immunosuppression and adherence. A normal ALT does not exclude fibrosis; cirrhosis with detectable HBV DNA generally warrants treatment.
