Adult HCM is otherwise unexplained maximal LV wall thickness ≥15 mm; 13–14 mm can qualify with a pathogenic variant, an affected first‑degree relative or a compelling phenotype. First exclude the phenocopies: hypertension, AS, athlete's heart, amyloid, Fabry disease.
