TBM presents with subacute headache/fever, cranial neuropathy, altered sensorium, hydrocephalus, basal enhancement, infarcts, hyponatraemia or immunosuppression. Send large-volume CSF for Xpert MTB/RIF Ultra and mycobacterial culture; negative tests do not exclude TBM.
