In ILD, the problem is not mainly inside the large air pipes. It is in the delicate walls around the air sacs. Common clues are slowly worsening breathlessness, a dry cough, fine "Velcro" crackles, clubbing, and tests showing small stiff lungs with poor oxygen transfer.
