Pulmonary hypertension is confirmed by right-heart catheterization, not by echo alone. The current haemodynamic definition is mean pulmonary artery pressure above 20 mmHg at rest. Pre-capillary PH also needs normal wedge pressure and high pulmonary vascular resistance.
