The reasoning: a calcium loop and a phosphate loop
Plasma calcium (total ~2.2–2.6 mmol/L; 8.5–10.5 mg/dL): ~50% ionised (1.15–1.35 mmol/L), ~40% protein‑bound (albumin), ~10% complexed. Alkalosis increases albumin binding → ↓ ionised Ca²⁺ (tetany with hyperventilation). Corrected calcium (mg/dL) ≈ total + 0.8 × (4.0 − albumin g/dL) — an estimate; measure ionised Ca²⁺ when it matters.
