CT head is the IOC (bone + brain). Canadian CT Head Rule / NICE: GCS < 15 at 2 h, suspected open/depressed or basal skull fracture, ≥ 2 vomits, age ≥ 65, anticoagulation, post‑traumatic seizure, focal deficit, dangerous mechanism, amnesia > 30 min. Skull radiographs have no routine role (except skeletal survey in suspected NAI).
