A 66‑year‑old man has become steadily more breathless over two years, with dry crackles at both bases. HRCT shows tiny stacked air cysts at the edges and bottoms of both lungs (honeycombing) with widened airways pulled open by scarring. This pattern points strongly to idiopathic pulmonary fibrosis.
