A 40‑year‑old man keeps bumping into door frames on both sides. Eye tests show loss of the outer halves of his vision (bitemporal hemianopia). MRI shows a mass rising out of the pituitary fossa with a "snowman" waist, pressing on the optic chiasm: a pituitary macroadenoma.
