A 35‑year‑old woman has had difficulty swallowing both solids and liquids for a year and regurgitates undigested food at night. Her barium swallow shows a very dilated oesophagus tapering smoothly at the bottom into a narrow bird's beak: achalasia. The lower oesophageal sphincter fails to relax.
