A 40‑year‑old woman has severe right upper pain after a fatty meal, with fever. Ultrasound shows stones, a thick oedematous gallbladder wall and fluid around it. Pressing the probe over the gallbladder makes her wince (sonographic Murphy's sign): acute calculous cholecystitis.
