A spectrum from tremor/anxiety to seizures and delirium tremens after stopping or cutting down alcohol in dependence. Benzodiazepines and thiamine are the mainstays; delirium tremens can be fatal.
Alcohol-dependent people who stop or reduce intake; severe withdrawal is more likely with prior DTs/seizures, heavy use and comorbidity.
Prevent/treat withdrawal (seizures, DTs)
Caution: Use shorter-acting agents in severe liver disease.
Prevent Wernicke's encephalopathy
Caution: Always give thiamine before glucose.
