The reasoning: pulsatile GH, stable IGF‑1
GH (191 aa) is secreted in pulses, maximal in slow‑wave sleep. Stimuli: GHRH, ghrelin, hypoglycaemia, fasting, exercise, stress, arginine, α‑adrenergic, oestrogen. Inhibitors: somatostatin, IGF‑1, GH (short loop), hyperglycaemia, free fatty acids, obesity, glucocorticoid excess, hypothyroidism. GH acts via GHR–JAK2–STAT5.
