In real life
In the OPD, a patient stands facing a plate, hands on hips, and takes a big breath in: a PA chest film. In the ICU, a ventilated patient can't stand, so a portable machine takes an AP film lying down. When the intern compares them, the heart looks bigger on the ICU film. The heart hasn't grown; the view has changed.
