In real life
Two breathless patients in the medical ward. One has fluffy white patches spreading out from both hila like bat wings: fluid in the air sacs from heart failure. The other has a fine net‑like pattern at both lung bases, worse over months: scarring of the lung framework (pulmonary fibrosis). Same symptom, opposite patterns.
