A man who drinks heavily was found unconscious after a binge. A week later he has fever and foul‑smelling sputum. His film shows a round cavity with a straight air‑fluid level in the back of the right lung. He inhaled mouth bacteria while unconscious and developed a lung abscess, in the classic place for aspiration.
