A 60‑year‑old with a past heart attack comes in breathless at night. His film shows a large heart, prominent upper lobe vessels, fine horizontal lines at the lung edges, fluffy shadows around both hila, and small effusions. All the stages of heart failure on one film. After diuretics, the next day's film is much clearer.
