The reasoning: mechanism of harm decides urgency
Waldenström macroglobulinaemia is lymphoplasmacytic lymphoma with IgM monoclonal protein. The large IgM molecule can cause hyperviscosity, neuropathy, cryoglobulinaemia and bleeding. AL amyloidosis is misfolded light-chain deposition in organs, especially kidney, heart and nerves. POEMS requires polyneuropathy and a monoclonal plasma-cell disorder, with a wider syndrome pattern.
